各学院、中心、实验室：

根据《教育部办公厅关于开展加强高校实验室安全专项行动的通知》（教科 厅函[2021]38号）以及本校实验室安全责任体系的要求，将进一步细化学校、学院、实验室三级联动的实验室安全管理责任体系，明确各级安全责任。《西湖大学实验室安全责任书》将进行逐级签订，学校与学院签订、学院与实验室负责人签订、实验室负责人与实验室人员签订，并实行“一年一签”。目前，2021年签订的实验室安全责任书即将到期，现启动2022年《西湖大学实验室安全责任书》签订工作。

一、责任期限为：2022年1月1日至2022年12月31日。

二、签订方式：

（1）学校与学院签订：一式两份，学校、学院各存档一份；

（2）学院与实验室负责人签订：一式三份，学院、实验室、学校各存一份；

（3）实验室负责人与实验室人员签订：一式两份，学院、实验室各存一份，学校存档电子版。

三、责任书模板：

（1）学院可根据学科情况，修改附件1《西湖大学实验室安全责任书-实验室负责人》参考模板，并组织签订。如果无修改，请填写附表3并反馈实验室与科研设施部，由学校统一制作标准版责任书发放学院组织签订。请各学院在2021年12月30日前反馈附表3。

（2）实验室负责人可根据实验室情况，修改附件2《西湖大学实验室安全责任书-实验室人员》参考模板，并组织签订。请各学院在2022年1月1日前反馈西湖大学实验室安全责任书-实验室人员》签订的扫描件。

All schools, centers & labs：

According to the relevant documents of the Ministry of Education and the requirements of Westlake University, the statement of responsibilities for laboratory safety will be signed level by level in 2022. The specific signing requirements are as follows:

Ⅰ. Period of Responsibilities: From January 1, 2022 to December 31, 2022.

Ⅱ. Signing requirements:

(1) Westlake University Statement of Responsibilities for Laboratory Safety -School will be signed between the university and the school, this statement is in triplicate, with the university and the school holding each copy respectively

(2) Westlake University Statement of Responsibilities for Laboratory Safety - Principal Investigator (PI) will be signed between the school and PI, this statement is in triplicate, with the school and the PI holding each copy respectively, and the university keep one copy.

(3) Westlake University Statement of Responsibilities for Laboratory Safety -Laboratory Personnel will be signed between PI and the laboratory personnel, this statement is in duplicate, with the laboratory and the school holding each copy respectively, and the university keep scanned version.

Ⅲ. Template

The school can modify the template of Westlake University Statement of Responsibilities for Laboratory Safety - Principal Investigator (PI), and organize to sign. If you need standard templates, please fill in and feedback the attached form to us.

PI can modify the template of Westlake University Statement of Responsibilities for Laboratory Safety - Laboratory Personnel, and organize to sign.
