各学院（单位）：

为切实加强实验室安全管理，提高实验室安全管理的规范性、有效性，根据教育部《高等学校实验室安全规范》（教科信厅函〔2023〕5号）、《浙江省教育厅办公室关于开展高校实验室安全分级分类管理工作的通知》（浙教办安〔2022〕34号）、《浙江省教育厅办公室关于启用高校实验室安全数治平台的通知》（浙教办函〔2023〕99 号）（附件1-3）等文件要求，学校将开展2023年度实验室安全分级分类工作，具体安排如下：

一、工作范围

分级分类工作的范围为我校开展实验教学、科学研究、技术研发等活动的各类实验场所以及配套的附属场所。

二、总体要求

 以最小物理建筑空间且相对独立的实验场所（实验室房间）为单元，根据《浙江省高等学校实验室危险源分级分类划分标准（试行）》（详见附件2），将实验场所内的危险源进行辨识、分级评估，进而确定实验场所安全风险等级，并在此基础上采取针对性的安全管理措施，进行风险分级防控，保障实验室安全。

三、工作安排

1.实验室辨识、填报分级分类信息（2023年6月25日17:30前）

实验室开展危险源排查、辨识，并在实验室综合管理系统（https://lab.westlake.edu.cn/）填报分级分类信息（系统操作手册见附件4）。学校也将于6月20日组织《实验室危险源辨识和信息填报》专题培训，指导实验室查漏补缺。

2.学院（单位）审核分级分类结果（2023年6月30日17:30前）

各学院（单位）督促指导实验室进行危险源辨识、填报，并于2023年6月30日17:30前完成本单位的分级分类信息审核工作。

3.学校复核分级分类结果并上报省教育厅（2023年7月10日17:30前）

实验室与科研设施部复核学院（单位）提交的分级分类结果，并于2023年7月10日17:30前，将全校实验室分级分类结果汇总上报至浙江省实验室安全数治平台。

4.实验室分级分类动态管理（长期）

实验室房间地址、安全员等基本信息以及危险源发生变化时，实验室应及时对分级分类信息、应急预案进行调整，并重新提交审核。

各学院（单位）应根据《浙江省高等学校实验室安全分级分类管理基本要求》（详见附件2）对实验室进行分级分类管理，开展相应频次的院级检查，并做好台账记录。

学校将根据省教育厅要求，每年年初对全校实验室安全分级分类进行年度再评估并上报。

四、相关要求

1.各学院（单位）要高度重视此项工作，认真组织实验室开展危险源辨识，做到分级分类工作全底数、无遗漏。

2.各学院（单位）要根据危险源分级分类结果，督促实验室配备相应防护设施设备，落实准入培训、实验室自查、应急预案与演练等分级分类管理要求。

All Schools(Units)：

According to the requirements of the superior administrative authorities (Appendix A-C), Office of Laboratories and Research Equipment plans to conduct a lab risk assessment in the following days. The relevant matters are hereby notified as follows:

Scope of assessment：

All the laboratories in our university which used for teaching or research, including gas cylinder room and other ancillary places, should be assessed.

General requirements:

The hazards in the smallest room of laboratories, should be identified and classified based on Classification Standard of Laboratory Hazards for Institutions of Higher Education in Zhejiang Province (Trial). (Refer to Appendix B.)

The laboratories should implement related safety management requirements after the assessment, according to the Basic Requirements for Laboratory Safety Classification Management in Institutions of Higher Education in Zhejiang Province. (Refer to Appendix B.)

Assessment schedule:

Self-assessment: by 17:30, Jun 25^th, 2023.

School assessment: by 17:30, Jun 30^th, 2023.

University assessment: by 17:30, July 10^th, 2023.

Re-assessment: If lab hazards changed, re-assessment should be conducted in time. If not, re-assessment will be conducted by the University annually.

Note:

-Please attach great importance to this activity.

-The assessment will be conducted via the laboratory integrated management system (https://lab.westlake.edu.cn/). The operation manuals are attached as appendix D&E.

-There will be a lecture on laboratory hazard identification and self-assessment on Jun 20th. Please arrange relevant personnel to attend.
