根据杭州市卫生健康委员会的通知要求，学校将从即日起开展2024年病原微生物实验室风险评估工作。相关事项如下：

一、评估内容

依据《实验室生物安全风险自评表》（附件1）对全校病原微生物实验室（BSL-2、BSL-1实验室）开展风险评估。

二、工作安排

1、实验室自评估：

2024年7月17日前，实验室完成病原微生物盘库、病原微生物实验活动风险再评估（2024年评估批准的除外）以及实验室生物安全风险评估，并将《实验室保存的病原微生物盘库汇总表》（附件2）、《病原微生物实验活动风险评估表》（附件3）、《实验室生物安全风险自评表》发给学院（单位）。

2、学院（单位）复核：

2024年7月22日前，学院（单位）复核实验室评估情况后，将评估资料汇总并发送至实验室与科研设施部邮箱：share@westlake.edu.cn。

3、学校评估：

2024年7月30日前，学校评估并上报上级主管部门。

三、其他

请学院（单位）督促相关实验室每月对实验室保存的病原微生物进行盘库，并由学院（单位）汇总后于每月10日前将本单位的盘库汇总表发我部邮箱。

Related schools & units：

According to the requirements of the relevant higher authorities, the biosafety risk assessment activity will be conducted in the following days. The relevant matters are as follows:

Assessment items:

The labs relate to pathogenic microorganisms, such as BSL-2 and BSL-1 labs, shall conduct the assessment basis on biological safety risk self-assessment form (appendix A).

Schedule:

Labs self-assessment: Summary form of stored pathogenic microorganisms (appendix B), risk assessment form for the experimental activities of pathogenic microorganisms (appendix C), and biological safety risk self-assessment form should be completed and submitted to school by July 17, 2024. The newly approved pathogenic microorganisms experimental activities in 2024 do not need to be resubmitted.

Schools assessment: Schools should collect and review all the assessment documents，then please send to share@westlake.edu.cn by July 22, 2024.

University assessment: University will review and submit the assessment documents by July 30, 2024.

Note:

The stored pathogenic microorganisms should be inventoried monthly by the safety coordinator of lab, and the summary form of stored pathogenic microorganisms should be sent to share@westlake.edu.cn before the tenth of each month by safety coordinator of school.
