各学院（单位）、实验室：

为贯彻落实《高等学校实验室安全规范》（教科信厅函〔2023〕5号）及学校有关安全工作责任制的要求，强化学校、学院（单位）、实验室三级联动的实验室安全管理责任体系，明确各级安全目标、落实安全责任，学校组织各学院（单位）逐级签订《西湖大学实验室安全责任书》，并实行“一年一签”。目前，2025年签订的实验室安全责任书即将到期，现启动2026年《西湖大学实验室安全责任书》签订工作。

一、责任期限为：2026年1月1日至2026年12月31日。

二、签订方式：

（1）学校与学院（单位）签订：一式两份，学院、学校各存档一份；

（2）学院（单位）与实验室负责人签订：一式三份，学院、实验室、学校各存一份；

（3）实验室负责人与实验室人员签订：一式两份，学院（单位）、实验室各存一份，学校存档电子版。

三、责任书模板：

（1）附件1《西湖大学2026年度实验室安全责任书-学院（单位）》,纸质版已发放至各学院（单位）安全管理人员。

（2）附件2《西湖大学2026年度实验室安全责任书-实验室负责人》参考模板，学院（单位）可根据本单位的实际情况修改此模板并组织签订。

（3）附件3《西湖大学2026年度实验室安全责任书-实验室人员》参考模板，实验室负责人可根据实验室的实际情况修改此模板并组织签订。

由各学院（单位）安全管理人员组织本学院（单位）的责任书签订工作，并于2026年1月20日前将签订后的学院（单位）、实验室负责人责任书（纸质版），实验室人员责任书（扫描版）报送给实验室与科研设施部。

All schools, units & labs：

According to the relevant documents of the Ministry of Education and the requirements of the University, the statement of responsibilities for laboratory safety will be signed in 2026. The specific requirements are as follows:

Ⅰ. Period of Responsibilities: From January 1, 2026 to December 31, 2026.

Ⅱ. Signing requirements:

(1) Westlake University Statement of Responsibilities for Laboratory Safety -School will be signed between the university and the school, this statement is in duplicate, with the university and the school retaining each copy respectively.

(2) Westlake University Statement of Responsibilities for Laboratory Safety - Principal Investigator (PI) will be signed between the school and PI, this statement is in triplicate, with the school and the PI retaining each copy respectively, and the university keep one copy.

(3) Westlake University Statement of Responsibilities for Laboratory Safety -Laboratory Personnel will be signed between PI and the laboratory personnel, this statement is in duplicate, with the laboratory and the school retaining each copy respectively, and the university keep scanned version.

Ⅲ. Template

The school may modify the template of Westlake University Statement of Responsibilities for Laboratory Safety - Principal Investigator (PI), and organize to sign.

PI may modify the template of Westlake University Statement of Responsibilities for Laboratory Safety - Laboratory Personnel, and organize to sign.

The school (unit) safety secretaries organize relevant personnel to sign responsibility statement. Please submit the signed statements before January 20, 2026.
