各学院（单位）：

为切实加强实验室安全管理，提高实验室安全风险防范的针对性和有效性，根据上级部门要求，学校将开展2026年度实验室安全分级分类工作，具体安排如下：

一、工作范围

分级分类工作的范围为我校开展实验教学、科学研究、技术研发等活动的各类实验场所以及配套的附属场所。

二、总体要求

以最小的实验室房间或相对独立的实验场所为单元，根据《浙江省高校实验室危险源分级分类划分参照表》（详见附件1），将实验场所内的危险源进行分类辨识、分级评估，进而确定实验场所安全风险等级，并在此基础上采取针对性的安全管理措施，进行风险分级防控，保障实验室安全。

三、工作安排

1. 实验室排查辨识并填报（2026年3月20日17:30前）

实验室根据附件1的2025版分级分类标准开展危险源排查、辨识，并在实验室综合管理系统（https://lab.westlake.edu.cn/）填报分级分类信息（系统操作手册见附件2和附件3）。学校也将组织实验室危险源辨识专题培训（具体通知后续发布），指导实验室查漏补缺。

2. 学院（单位）审核（2026年4月3日17:30前）

各学院（单位）结合月度实验室安全检查、专项检查等，现场核实实验室分级分类情况，于2026年4月3日17:30前完成本单位的分级分类信息审核工作。

3. 学校复核上报（2026年4月10日17:30前）

实验室与科研设施部复核学院（单位）提交的分级分类结果，并根据省厅要求，将全校实验室分级分类结果上报至浙江省实验室安全数治平台。

4. 实验室分级分类动态管理（长期）

实验室房间地址、安全员等基本信息以及危险源类型与数量等发生变化时，实验室应及时重新进行危险源识别和安全风险评估，重新判定实验室安全风险等级，并相应调整应急预案，重新提交审核。

各学院（单位）应根据《浙江省高校实验室安全分级管理要求参照表》（附件4）对实验室进行分级分类管理，开展相应频次的院级检查，并做好台账记录。

学校将根据省教育厅要求，每年年初对全校实验室安全分级分类进行年度再评估并上报。

四、相关要求

1. 各学院（单位）要高度重视此项工作，请组织所有实验室重新完成危险源辨识，做到分级分类工作全底数、无遗漏。

2. 各学院（单位）要根据危险源分级分类结果，督促实验室配备相应防护设施设备，落实准入培训、实验室自查、应急预案与演练等分级分类管理要求。

All Schools(Units)：

According to the requirements of the superior administrative authorities, Office of Laboratories and Research Equipment plans to conduct a lab risk assessment in the following days. The relevant matters are hereby notified as follows:

I. Scope of assessment：

All the laboratories in our university which used for teaching or research, including gas cylinder room and other ancillary places, should be assessed.

II. General requirements:

Based on the smallest single lab room or a relatively independent experimental area as a unit, the hazard sources should be identified and classified, and then determine the safety risk level of the experimental place according to the Reference Table of Laboratory Hazards source Classification for Institutions of Higher Education in Zhejiang Province. (Appendix A.)

The laboratories should implement related safety management requirements after the assessment, according to the Reference Table of Requirements for Laboratory Safety Classification Management in Institutions of Higher Education in Zhejiang Province. (Appendix D.)

III. Assessment schedule:

Self-assessment: by 17:30, March 20, 2026.

School assessment: by 17:30, April 3, 2026.

University assessment: by 17:30, April 10, 2026.

Re-assessment: If lab hazards changed, re-assessment should be conducted in time. If not, re-assessment will be conducted by the University annually.

Note:

-Please attach great importance to this activity. Each lab should conduct self-assessment in time.

-The assessment will be conducted via the laboratory integrated management system (https://lab.westlake.edu.cn/). The operation manuals are attached as appendix B&C.

-There will be a lecture on laboratory hazard sources identification and self-assessment. You can wait for the follow-up notice for details. Please arrange relevant personnel to attend.
