根据杭州市卫生健康委员会的通知要求，学校将从即日起开展2026年病原微生物实验室风险评估工作。相关事项如下：

一、评估内容

依据《实验室生物安全风险自评表》（附件1）对全校病原微生物实验室（已备案的BSL-2实验室）开展风险评估。

二、工作安排

1、实验室自评估：

2026年7月20日前，BSL-2实验室完成实验室生物安全风险评估，并将《实验室生物安全风险自评表》提交至学院（单位）。如BSL-2实验室还涉及人体生物样本的，还需填写《实验室人体生物样本盘库汇总表》《实验室人体生物样本处理情况调查表》（附件2-3），一并提交至学院（单位）。

2、学院（单位）复核：

2026年7月24日前，学院（单位）复核实验室评估情况后，汇总评估相关资料并发送至实验室与科研设施部邮箱share@westlake.edu.cn。

3、学校评估：

2026年7月30日前，学校评估并上报上级主管部门。

三、其他

请学院（单位）组织BSL-2实验室，全面落实实验室风险识别、评估与控制工作。加强实验活动管理，涉及病原微生物操作的实验活动须提前完成风险评估和审批，并在相应等级生物安全实验室开展；加强实操培训和应急处置演练，每年至少开展一次实验室应急处置演练，并及时调整应急处置预案；加强病原微生物、人体生物样本全链路管理，每月10日前将《实验室保存的病原微生物盘库汇总表》《实验室人体生物样本盘库汇总表》发送至实验室与科研设施部邮箱share@westlake.edu.cn。

According to the requirements of the relevant higher authorities, the biosafety risk assessment will be conducted in the following days. The relevant matters are as follows:

Assessment items:

The labs relate to pathogenic microorganisms, such as BSL-2 labs, shall conduct the assessment based on Biological Safety Risk Self-Assessment Form (Appendix A).

Schedule:

Labs self-assessment: All registered BSL-2 labs should submit the Biological Safety Risk Self-Assessment Form to school by July 20, 2026. If BSL-2 labs relate to human biological samples, the Summary Form of Human Biological Samples (Appendix B), and the Investigation Form of Human Biological Samples Processing (Appendix C) should also be filled out and submitted to the school together.

Schools assessment: Schools should collect and review all the assessment documents，and send them to share@westlake.edu.cn by July 24, 2026.

University assessment: University will review and submit the assessment documents by July 30, 2026.

Note:

Schools are requested to organize BSL-2 labs to fully implement laboratory risk identification, assessment, and control measures. Strengthen the management of laboratory activities. Experiments involving pathogenic microorganisms must undergo risk assessment and approval in advance and be conducted in biosafety laboratories of the appropriate level. Enhance practical training and emergency response drills. Conduct at least one laboratory emergency response drill annually and promptly update emergency response plans. Strengthen end-to-end management of pathogenic microorganisms and human biological samples, and the Summary Form of Stored Pathogenic Microorganisms and the Summary Form of Human Biological Samples should be sent to share@westlake.edu.cn before the 10th of each month by safety coordinator of school.
